Optique is located in the Middleboro Crossing shopping plaza in central Middleboro, MA, near Job Lot. We serve our local community, as well as assisted living facility residents. Our shop provides prescription eye wear, sunglasses, custom work, and “over-the-counter readers” with rapid turnaround, often in just a few days. Our optometrists are accepting new patients for eye exams and contact lens fittings. In addition, we provide repairs, adjustments, nose pad replacement, and tune-ups to eye wear. Massachusetts law requires a valid prescription generated within one year, and we are happy to contact your eye doctor for you to have your prescription faxed to our office.

STEPHEN D. MORRIS, O.D.

Dr. Stephen Morris received his Bachelor degree from University of Massachusetts, Amherst and subsequently graduated with honors from the New England College Of Optometry in 1981. Dr. Morris enjoys outdoor activities such as running and is an active member of his local community.

Leigh C. Kozak, O.D.

Dr. Leigh Kozak received her Optometry Degree from the prestigious University of California, Berkeley in 1994. Dr. Kozak brings over 25 years of experience in the field of medical eye care. In addition to her experience in contact lens and ocular surface disease, Dr. Kozak is well known for her ability in helping patients succeed in complex contact lens cases and complex eye glass prescriptions. She is also highly skilled in the management of cataract and LASIK patients. She resides in Raynham and is an active member in the local community, a former collegiate swimmer, she also enjoys gardening in her free time.

HALEY CLOUDMAN

Haley has grown up in Middleboro and recently moved back to the area after being away for 6 years traveling the country with her husband due to the Military. Haley was a previous employee here at Optique starting when she was in high school. Haley has been in the optometry/optical field for 10 years and 4 of those years has been as the acting optician. Haley enjoys spending time with family and friends. Loves going to the Cape each year and would love to travel more in the future.

CRISTINA MENEZES

Cristina has been a part of Optique of Middleboro for six years. Cristina’s favorite part of her job is building relationship with patients. Cristina’s duties involves greeting and checking out patients, insurance verification, billing and coordinating care with our specialists’ offices. Cristina also assists the doctor with providing excellent eye care. Our staff is always willing to go above and beyond for our patients, and she is proud to be a part of our team.
